it's probably fused to your skin somewhere. No matter how you get it out, it's not gonna be pleasant.
But yeah, forceps or pliers would be the best bet. I'd say use some piercing cleaning solution to kind of lube it up. Maybe try twisting it around first? If you can turn it, then it's not fused and I'm not sure why you couldn't get it out.

I don't think the piercing healed on the barbell. She's had it for almost two years, she would have noticed the irregular healing process in the first two months.

I hope you have enough breathing space on that bar..
Use a hemostat [tweezer forcep] or a needle nose pliers [make sure It's clean] to clamp on the bar to get a secure grip.
Then loosen with your free hand, use rubber gloves or one of those jar opening rubber mat thing. Those work really good. Try it on both sides, maybe the ball was screwed in the wrong way, It probably set in like that and locked itself. Goodluck!
